Vaccine Administered & VFC Eligibility Report (New Hampshire)
This report details the number of vaccines administered based on age range and VFC eligibility category. Dose number is not displayed on this report; only the number of vaccines administered by age group and VFC eligibility category are displayed.
This report can be scheduled. Click the Schedule button to schedule the report to run during off-peak hours. See Schedule a Report for more information.
Run the Report
To run this report, click the Reports > State Reports > Vaccine Administered and VFC Eligibility Report link. The Vaccine Administered and VFC Eligibility Report page opens. Enter the criteria and click Create Report. The report displays in a separate browser tab and can be printed using the browser's print function. To export the report, click Export Report.
The report parameters are as listed below:

Person Completing Report |
Enter the name of the person completing the report. |

Phone Number |
Enter the person's phone number. |
|
Vaccination Date Range |
To limit the report to a specific vaccination date range, enter the From and To dates, after which the checkbox is automatically selected. |

Organization (IRMS) / Organization (IRMS) Group / Do Not Limit |
To limit the report by an Organization (IRMS) or Group, select it from the related drop-down list, after which the radio button is automatically selected. Do Not Limit is the default. |

Facility / Facility Group / Do Not Limit |
To limit the report to a specific Facility or Facility Group, select it from the related drop-down list, after which the radio button is automatically selected. Do Not Limit is the default. |

VFC PIN |
To limit the report to a specific VFC PIN, select it from the drop-down list (after which the radio button is automatically selected). |

Region |
To limit the report to a specific region, enter the region number. |

Vaccines |
To limit the report to specific vaccines, select one or more vaccines in the Unselected list and click Add to move them to the Selected list (click Add All to add all of them at once). To remove a vaccine, select it in the Selected list and click the Remove button to move it back to the Unselected list. |

Age Range |
To limit the report to one or more age ranges, select them in the list. |

Show age breakdown in exported report |
Select this option if you are going to export the report and want to display the age breakdown. |

VFC Status |
To limit the report to one or more VFC statuses, select them in the list. |
|
Funding Type |
To limit the report to a specific funding type, select it in the drop-down list. Otherwise, select All. |
|
All Active Providers |
Select this option to include all active providers in the report. |
|
Current Provider Agreement Only |
Select this option to include only providers that have a current provider agreement. |

View By |
Select the option for how you want the report to be displayed by:
